STUDENT COMPUTER AND INTERNET USE ACKNOWLEDGMENT FORM

No student shall be allowed to use school computers until the student has signed and returned the Student Computer Acknowledgment form. No student shall be allowed to use the Internet until both the Student Internet Acknowledgment and Parent/Guardian Internet Acknowledgment forms have been signed and returned to the school.
